Sensitivity and specificity of current diagnostic guidelines in children with macrophage activation syndrome complicating systemic juvenile idiopathic arthritis

چکیده
Background Early diagnosis of macrophage activations syndrome (MAS) in systemic juvenile idiopathic arthritis (sJIA) may be challenging because it may mimic the clinical features of the underlying disease or be confused with an infectious complication. However, the diagnostic value of the guidelines for hemophagocytic lymphohistiocytosis (HLH) (1) or sJIA-associated MAS (2) has seldom been examined.

Macrophage Activation Syndrome as the First Presentation of Juvenile Idiopathic Arthritis
Macrophage activation syndrome (MAS) is a rare feature of rheumatic disorders in children and adolescence and its presentation as the first symptom of rheumatic disorders is very infrequent. A 9-year-old girl, in whom MAS developed, was admitted to our Hospital in Tehran, Iran. She suffered from high grade fever and rash followed by multiple joint swelling months afterwards. Bone marrow aspira...

Background :Juvenile Idiopathic Arthritis (JIA) is one of the most common chronic rheumatic diseases in children with unknown etiology and pathogenesis. It also has no diagnostic test and its clinical diagnosis is made through ruling out other types of arthritis. The aim of this study was to evaluate the level of ADA (Adenosine Deaminase) in the serum of JIA patients and to compare it with th...

OBJECTIVE To compare the capacity of the 2004 diagnostic guidelines for hemophagocytic lymphohistiocytosis (HLH-2004) with the capacity of the preliminary diagnostic guidelines for systemic juvenile idiopathic arthritis (JIA)-associated macrophage activation syndrome (MAS) to discriminate MAS complicating systemic JIA from 2 potentially confusable conditions, represented by active systemic JIA ...

We read with deep interest the paper by Ravelli et al, who developed new criteria for the classification of macrophage activation syndrome (MAS) in patients with systemic-onset juvenile idiopathic arthritis (So-JIA). The hemophagocytic lymphohistiocytosis (HLH) diagnostic guidelines have been successfully used in the management of primary HLH, but there have been several limitations to their us...

OBJECTIVES The purpose of our study was to detect early clinical and laboratory signs that help to discriminate macrophage activation syndrome (MAS) from active systemic juvenile idiopathic arthritis (SJIA) without MAS. METHODS Our retrospective study was based on reviewing the medical charts of the children admitted to the rheumatology department with active SJIA and definite MAS (n = 18) an...
